Whack - you may be right but my understanding of the "time out" situation was that time was back in when the umpire says so. In most cases, that is when the coach is back in the dugout and the pitcher is on the mound, not necessarily on the rubber. Now I gotta go get a rule and look it up.
And to your other point, my kids have always known not to take their lead until they see the pitchers foot on the rubber. We taught them that when we taught them how to take leads, secondary leads and steal.
